Pharmacists in Wales

  • Must have a minimum of six months experience working within a UK Pharmacy
  • Must be set up as Self-employed
  • Enhanced DBS check (must be subscribed to DBS Update Service) by 1st June 2017 - (this must be a copy of the paper certificate; we cannot accept electronic copies)
  • Update Service Enabled DBS
  • Proof of Identity - Passport
  • Proof of NI
  • Immigration status/visa and permit number (If applicable)
  • Any Qualifications or Accreditations
  • MUR/DMS certificate
  • NMS form/certificate
  • Professional Indemnity Insurance
  • WCPPE Safeguarding level 2, by 1st June 2017 -
  • WCPPE Choose Pharmacy, by 1st June 2017
  • Completed NADEX Listing Form & Statement, by 1st June 2017

England

  • CPPE Safeguarding level 2

ACCURACY CHECKING TECHNICIANS & GPhC REGISTERED TECHNICIANS

  • Must have a minimum of six months experience working within a UK Pharmacy
  • Must be set up as self-employed
  • Pharmacy NVQ level 3 certificate
  • Proof of identity – Passport
  • Proof of National Insurance
  • Enhanced DBS check (this must be a copy of the paper certificate; we cannot accept electronic copies) subscribed to DBS Update Service or PVG for Scotland
  • Immigration status/Visa (if applicable)
  • Professional Indemnity Insurance
  • CPPE Safeguarding children & vulnerable adults level 2 (2017)

DISPENSING ASSISTANTS

  • Must have a minimum of six months experience working within a UK Pharmacy
  • Must be set up as self-employed
