New Directions Social Care is looking for compassionate, dedicated and reliable Adult Support Workers across the Swansea area.

Key Responsibilities
Deliver high quality, person centred support tailored to individual care plans
Support individuals with daily living activities, including personal care where required
Assist with daily living activities, including meal preparation and household tasks
Encourage and support individuals to access the community and participate in activities they enjoy
Administer or assist with medication
Build positive, trusting relationships with the individuals you support
Observe, monitor, and accurately record changes in individuals wellbeing
Complete daily records ad documentation to a high standard
Follow safeguarding procedures and report any concerns appropriately
Work collaboratively with colleagues and healthcare professionals to deliver high quality support
Maintain a safe, respectful, and inclusive environment at all times
Requirements
Essential:
Minimum 6 months’ experience in a support worker, care, housing support, hostel, mental health, or related role.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Flexible approach to working various shift patterns.
Commitment to promoting dignity, respect, and equality.
Enhanced DBS on the Update Service (or willingness to obtain one).
Right to work in the UK.
Desirable:
Experience working within supported living, homelessness services, hostels, mental health services, or substance misuse settings.
NVQ/QCF Level 2 or 3 in Health & Social Care or a related qualification.
Mental Health Awareness, Safeguarding, or Prevention and Management of Violence and Aggression (PMVA) training.
Full UK driving licence and access to a vehicle.
